As long as you're playing in DOSBox and you're not assigning mouse buttons/movements to your joystick, I actually recommand using the DOSBox internat mapper, which can be found when pressing ALT-F1 at any time.
With it, you can click on any keyboard command, then choose "add" and press whatever button/axis you want the command to be mapped to.
If you want to do more complex things I actually recommend "Pinnacle Game Profiler" instead of XPadder. It's a much more versatile program that has also a much more professional look and feel to it.
