I have this on CD, running under DOSbox, and I found that the following audio settings sound best:
Music: Roland
Effects: Soundblaster Pro
If you're not lucky enough to own an MT-32, you'll need a version of DOSbox that includes MT-32 emulation. I use the feature-filled build by Daum (google "dosbox daum"). The original graphics are a bit pixellated, but look great using an advanced shader like xBR. I recommend the 5xBR-v3.8a.fx shader for this (google it), which is a very good looking D3D shader at 1080 output.
Note: I have no idea how this comes configured from GOG, but I do recommend Daum DOSbox for all titles from GOG that use DOSbox emulation (except Pinball Fantasies, which uses a custom build of DOSbox).
