In my last run, I had an Arbalest in the 3rd spot and a Plague Doctor in the fourth. When I selected battlefield bandage with the Arbalest, only the people in the first, second, and third slot were selectable. The plague doctor could not be selected in the fourth spot. I ended up having to flee the mission early.

Is this a bug or a feature?

No that's not a bug. Battlefield bandage can only target characters in positions 1, 2 and 3. Check the skill again.
