Just like it says on the tin. I own the Perpetual edition and the GOG client shows Exile as being owned, but the DLC does not show up in-game. If I try to install it manually, the installer returns an error message saying that I do not have the latest base game update, despite the fact that I do in fact have the latest update - at least the latest one available over the GOG client. Suggestions?

I have the Perpetual edition as well.

I had to go into "Manage Installation -> Configure" for Cultist Simulator in Galaxy and enable The Exile DLC. After a small update it was available in-game.
