Just finished the game on "insane" and received this message after the credits:

"New difficulty mode unlocked. you can choose Gate of Madness difficuty when starting a new game".
Except I cannot.

When choosing to start a new game "insane" is still the highest difficulty. i also cannot chance it AFTER i actually started the new game.

anybody can help me?really looking forward to this new challenge.....
