I can't seem to get it right - I build a reservoir adjacent to a river, in the green view before construction it shows up as filled, but then it doesn't fill up with water. Is there any other condition??
It has to be very close to the water. If there is no water nearby (aka, 1-2 tiles distances), it won't fill. Straight line coast is the best place to put down reservoirs, but some missions may need you to transport the water to distant reservoirs, so make sure you've connected up with aqueducts.

In fact, I encountered a graphic bug. The reservoir seems empty, but it works... weird.

A graphic bug ? Looks like the same bug encountered with Pharaoh. Try restarting your PC, as that fixes the issue.

heh, I'm using Linux (Wine 1.4) and first time I started the game (without restarting my PC), everything seemed to work fine. No reservoir bugs at all.
I've had the same bug. The fountains and resevoirs appear empty when they aren't.

You can tell though by the preview image, if it shows it will be fill it means it should be if you build it there, even if you can't see the water
Make sure that it is a graphic bug and not what happens when you don't have enough people assigned to waterworks. Reservoirs/fountains/baths will work intermittently if only a fraction of the people needed is assigned (and appear empty when it is not working).
