If you want your game to crash, then do the following:
Go to the piano, and sit on the chair. Wait till the song starts, then you can leave. Staying till the song is over, ensures the bug will go off.
Next, go through the hallway, till you reach the book case.
Now, read the book.
Congrats, you just broke the game, and an error report will pop up.

it's not the piano, what triggers the bug is reading books. Make sure you have saved BEFORE reading the books because you have a slight chance to experience the crash. It happens on PS4 and all other consoles as well. Hopefully it gets patched. :)
