Just curious to see what everyone's favorite level is. Mine is Dark Carnival. I love the theme, scary music, and all of the secrets within the mini-games at the carnival :)
It's very hard to choose, but if I have to pick only one, it would be The Haunting (E2M5). Genuinely creepy, isolated, alienated atmosphere. It was so unnerving I almost craved for a cultist enemy just to finally see another human being... :)

I loved the siege! The sirens, airplanes overhead, constant explosions, it was pretty flippin awesome
The Sick Ward, as it looks like twisted hospital with all horrors and mess inside.

what the heck that seems to be EVERYONE's favorite level... lol that is one of my least favorite levels the sirens get really annoying and I don't like the level layout

I like many levels of the game, but if we talk about episodes the best one was "Even death may die", no doubt about that, best atmosphere and level design, and the worst one was Post Mortem the one made for the Plasma Pack, they designed new enemies, items, textures... But the level design and atmosphere was much worse (some levels are really boring), it was a major disppointment for me, also It´s funny the fact that Cryptic Passage, expansion which doesn´t have a single new enemy, even a new single texture is much better as a game than the Plasma Pack, it wasn´t released here and I played it on the GOG version for the first time, awesome experience. It´s great to discover something new about one of your favourite games so many years later (13!!!!)

I haven't beaten the game yet but I have to say that almost all levels I've played so far are quite amazing. They do all seem quite inspired and original, even more than in Duke 3D or Shadow Warrior. I mean, everybody knows how the build engine games basically introduced those realistic and highly interactive environments but personally I feel that they weren't just the first, they were the best and I have the impression that (at least in terms of level design) Blood is the best of the three.

If I had to choose... I think so far I love the Desecrated Church most. I don't really know why. It's just so incredibly haunting to me.

Out of the levels I've played (I still haven't finished the game), that one is my favorite as well, for the very same reasons. It was surprising for me to feel that creeped out and paranoid in a 90s FPS :D
All the levels are great in my opinion, but I genuinely love Dark Carnival and the Overlooked Hotel levels the most.
First two levels in episode 2 and the overlooked hotel in episode 2.
I like the Overlooked Hotel and the Haunting, because they are traditional horror levels. I also like Phantom Express and Steamboat.

Sorry for the necro.

The first Blood had so many brilliant levels its hard to pick one. The one that will always stand out for me was "In The Flesh" like literately! Never I had seen before such a grotesque level in any game, the fact you are inside a huge living organism had some sinister vibe to it.

Close follow ups:

Dark Carnival and The Overlooked Hotel
Mall of the Dead, with Dark Carnival a close second.
"The Overlooked Hotel" is probably the winner for both design and aesthetics. It easily shows off Blood at it's best.

Other contenders would be;
Phantom Express
Dark Carnival
The Haunting

And of course "Cradle to Grave" is a classic first level.
