So I'm able to assign movement and strafing to WASD, which is all well and good, but I can't find any way to make "moving the mouse up/down" = "aim upwards/downwards". The Steam version of Shadow Warrior has that as the default control scheme, and that game runs on the same engine as BLOOD 1, so I know it's possible, I just don't know how

Unless you've remapped it, try pressing "U" to lock in mouse look. You might have to switch some settings in your blood.cfg file to invert the Y sensitivity if you aren't a fighter pilot. Also check the forums for the BMouse thread.
