It comes up with the Dosbox screen, then everything goes black and then it drops out.

Did you install to Program Files or someplace else like c:\games\Blood ?

Windows Vista/7/8 really lay on the default security setting in Program Files. Be sure to give your user account full control rights to that folder, and run as administrator. You might be better off reinstalling in a folder outside of PF.
