I'd try the following changes:
fullresolution=0x0 (or whatever you want if you have a CRT monitor)
output=openglnb
windowresolution=1024x768 (or pick whatever you want up to your desktop resolution)
memsize=64
scaler=none
core=dynamic
cycles=max
In-game I use 640x480 myself, but can run 800x600 relatively smoothly as well. My system: Core 2 Duo E8400 (3GHz) and Geforce 9600GT card.

This is getting kind of ridiculous. I've been trying to fix this for 4 days now, and I still cannot figure out how to. I've tried everyone's suggestions, and still gotten no reply from GOG after asking for support. One would think a modern computer with 2 processors could play an old game like Blood with no problems.

I don't understand you guys. resolution of 640x480 serves well and the framerate issue should be gone. Hmm I got the information that the memsize=63 schould be enough while 64 make it open to 128 and 63 gives 64. and in my case with an ATI X1900 and output=ddraw serves me better than output=opengl because of the colour calibration. Isn't Blood made for low reslution anyway or I'm wrong? I think that higher resolution only give an improvement if Blood got a damn sourceport. But now 640x480 will give the best framerates with a loss don't mention it.

How are you actually running the game? Did you copy the Blood directory from GOG's installed path, or are you navigating through DOS Box to the Blood directory under GoG's path?

Edit: Maybe if you try to install fresh from GOG your issues are gone. Did you try the Launcher from McTecman? There are god configs included. I hope this helps a little bit.

I have tried McTecman's launcher and every one of the mods lagged horribly except zBlood. I have DoxBOX .74 and GOG's One Unit Whole Blood. When I start Blood, it runs through DosBOX .74. I have an older brother who is computer smart and works for geek squad. His statement is that this game has a poor frame rate because it's "too old". I however, am not very computer smart. I have tried all of the suggested configurations and the results have not changed at all, even in 320x200 resolution. Are there any step by step instructions I can follow to fix this or am I ultimately fucked?

So do you use the shortcut that GoG creates?
1. Are you sure it runs through DosBox .74?
2. Are you sure it uses the config file you pasted (and modified with my changes)?
If you right-click on the shortcut and view Properties, what does it have in Target?

I'm very lucky to have a machine that can run this old games via dosbox. This old games unfortunately needs a very fast computer because dosbox is simulating everything what an old computer was and therefore takes a huge overhead in todays newer machines.
I have an old laptop with 1.4 ghz that isn't able to run blood via dosbox. One chance is, if you have old stuff so run dos on a second partition and take the game native. It will run like hell. But unfortunately dos doesn't serve newer grafikcards.
I think that Blood in dosbox will run from 2.5 GHz dual core upwards (with both cores used) in a mix from choppy to flawless with 640x480.
Really this is a shame that those old games needs this bunch of an asskick to run under dosbox.

Did you try a new install from fresh anyway?
Now i got it. In game under options it shows you 320x200 but that is normal. Did you change it to whatever 680x480 in the game options? I couldn't imagine that.

Install Instructions DosBox
At first run dosbox from scratch in your games folder: At the the command prompt in Dosbox type "config -writeconf dosbox.conf" and then press the "Enter" key.
Creating PC game config
Next: Type "exit" to exit Dosbox.
1 - Mount the game in cdrom drive or daemon tools letter F (F is only an example for your drive)
And Type this
* Mount C D:\(path from the gamesfolder) (D is only an example for your drive)
* Mount F F:\ -t cdrom
* F:
* Install (your game).exe
2 - in dosbox.conf add the lines under
[autoexec]
# Lines in this section will be run at startup.
# You can put your MOUNT lines here.
mount c d:\(path from the gamesfolder)
mount f f:\ -t cdrom
c:
cls
(your game name).exe
exit
.
.
.
finally launch dosbox by double click or make a link to it with the goal:
D:\(path of your gamesfolder)\DOSBox.exe -conf dosbox.conf -noconsole -c "exit"
where (D is only an example for your drive)

I don't have Blood through GOG, but I do have Redneck Rampage, which is pretty similar.
This is the shortcut GOG created:
Target: "C:\Program Files\GOG.com\Redneck Rampage Collection\DOSBOX\dosbox.exe" -conf dosboxRR.conf -noconsole -c "exit"
Start In: "C:\Program Files\GOG.com\Redneck Rampage Collection\Redneck Rampage\"
This is the shortcut I created to play the game the way I want to:
Target: "C:\Program Files\dosboxcvs-090704\dosbox.exe" -conf "C:\Dos Games\confs\73CVS\RedneckRampage.conf"
Start In: "C:\Program Files\dosboxcvs-090704"
Basically The target and Start In both need to reference the DosBox you want to use, and then you need to be sure to point to the correct .conf file.
This is the autoexec portion of the GoG-provided conf file for RR:

I removed the 'exit' command because I didn't want a separate shortcut for the Setup, and this lets me exit from the game into the commandline and run setup.exe myself.
I have had it reinstalled and I'm certain it is running through DosBox .74. I am still getting the same low framerate as usual. I've been able to slightly adjust it by tweaking some DosBox settings but have had no luck whenever I see an enemy. I really can't comprehend why I could play Blood with no problems on a computer made in 1997, but I can never get it to work right on a computer with 2 processors that can run CounterStrike just fine. Oh well, maybe I will get to play Blood again in 2017, by then it will be 20 years old and there won't be any excuses for poor performance.
