I have a bizarre issue where the mouselook in game is very jumpy and non-responsive. It's almost as if it's not reading all the inputs and is struggling trying to interpolate the signal!

I'm using DGVoodoo2 for the graphics driver but notice no improvement switching to Direct3D. Also I have Vsync on and have tried limiting FPS to 60 but it doesn't seem to help. Would really appreciate anyone that can figure this one out!

Trying to run the game in compatibility mode won't hurt and may solve your issue.
Right click on the game exe, select "Properties" then choose the "Compatibility" tab: there you can find the compatibility modes (Windows Xp SP3 should be the more appropriate, but try others too if necessary).

Ignore me, I somehow missed it was for Blood 2.

Thanks, this seems to have fixed it! Only issue is the mouse cursor seems to pop up briefly every 30 seconds or so though. No idea why that could be... at least it's playable though.
Also there are links on steam forums to patches that can make this game run smoother.
