There is a couple pieces of artwork in the extras but what's not included is any kind of manual or reference guide for what does what on the keyboard. A PDF scan of the games manual would be most acceptable.
In the meantime, just to help you out:

Movement: Arrow keys
Menu Select: Enter
Menu Back: Escape

Attack/Action/Magic/Inventory: Remappable
Forms, Items, Magic Selection: Remappable

controls can easily be looked up on the main menu in options or ingame via the menu button

and yes all are remappable ingame

and hate to tell ya this but not all manuels or reference guides during the time of the release of the game included any specific info on what keys do ingame.

A lotta guides from that time period typically would say things along the lines of, "To attack, press the attack key"

tho admittedly I personally havent read this games manuel

Except for movement which seems to be hardcoded to WASD or the arrow keys.
Easily fixed with an Autohotkey script though, which also allows for the mouse buttons to be assigned to whatever you want.
This is getting ridiculous, sure you *can* typically find the manuals online. But the whole point of GOG was that the game worked without any troubleshooting and included everything need to play.

In the 1990s it was assumed you would read the manual. The vast majority of 90s games should include their manuals. Yet with more than one new release - no manual.

Regardless of whatever licensing needed to be worked out to re-release the original Legacy of Kain this is another disappointing bare minimum release. Pass.
Well, you SORTA get the manual, in the game.chm file.
Uk manual:
