The title screen has no cursor or anything, and the text is all black in-game

The game has a circle icon like each time gog updates a game which means it should work but it doesn't.

How do I fix this? Is there a way from the settings in-game?

The game works best in rOpenGL (preferred) or in Glide mode with glidewrapper (somewhat slower and only 24bit colordepth), Direct3D isn't fully supported on modern hardware anymore which causes these sort of glitches.
