I didn't get the "Shalatuwar Fortress" and "Carnage at Shalatuwar" achievements. All previous achievements worked just fine. Anyone else has this problem?

New finds: I didn't get the "Kill the lava golem in the Abyss" and the "Kill the Vampyr in the Abyss" achievements for the first time. I tried again from a previous save, then they popped. I'm sure the Shalatuwar ones will unlock after the second time, but they should work normally at the first try.
Also, I managed to finish the game with "Heroic" rating, but I don't get the "Normal" and "Bold" achievements for that? That's kinda lame. Should I play deliberately bad just for these achievements?
