I have to use the rung on the door but I don't know how.

Well, once you pick the rung up by right clicking on it, scroll the pointer up to the top of the screen, and the inventory will appear. Then right click on the item (the rung, in this case) and after the cursor turns into the rung, left click on the door.
That should do it.

You get the manual with the game, you know. It's in the folder in the Start Menu.

RTFM as it were. I seem to recall being a bit lost when I started replaying it a few years back and it took me a hile to remember how to use items and even longer to remember where the hell the inventory was...

IIRC it's the left button to use/interact with an object and right click to examine.
