Sometimes I see 'unknown vehicle/mech' or '##t vehicle/mech' but it's not clear why sometimes this is visible and other times not.

It depends on the tactics skill of the mechwarrior who has enemy in sensor range. At level 4 he can identify the class (light/medium...), at level 7 he can identify the tonnage.

electronic warfare can also cloak units but that isn't common
