INT - Ramazith's Tower, Baldur's Gate city
STR - Catacombs beneath Candlekeep, chapter 6
WIS - Lady's House temple, Baldur's Gate city
WIS - Catacombs beneath Candlekeep, chapter 6
WIS - Durlag's Tower

Not unless you cheat (though I believe they do work if you do).

No, but you get similar boosts to stats in Watcher's Keep (Machine of Lum).
