My quick formations stopped working correctly. If I click on anything but the first formation, it only allows me to move in that formation once. It then automatically reverts back to the first formation and doesn't allow me to switch back, unless I tab out of the game. Sometimes I'll right click to bring up the menu of formation types and instead it will, by itself, change the formation to the horizontal line. I have no idea what's causing this. I'm playing on Vista and using Tutu.

Cool. Some mouse (and other hardware) drivers have a habit of freaking out on occasion. Rebooting is always a good idea when unusual, funky things happen. :)
