May include spoilers. I'm nearly at the end of the game (I think)

I was in vendigroth ruins and I have explored the different levels of the underground city. I saved just before I went into 4th vent. then I saw something round. I stepped on it and died instantly.

then I loaded the game from the last save point. clicked on the vent again but the game crashed.

I restarted the game, loaded the last save file and I realized that virgil's portrait is gone. There is only a blue rectangular shape where his portrait should be. I can't click on it. Event when I look at my other followers' inventories, when I come to Virgil it just kicks me out. I guess Virgil's file in the game folder is corrupted somehow.

now whenever I try to move to another level through the vent, the game crashes. It's unplayable right now. Please help.

I rather think that the save file is corrupted. Haven't you got a previous save file to load?
1) Did you download and install the UAP (Unofficial Arcanum Patch) linked to in the pinned thread at the top of this board? If not, do so immediately. This will not fix your corrupted save file, but it may help prevent any similar problem when you restart the game.

2) Have you been over-relying on the quicksave (F7) and quickload (F8) buttons? That's fine, but be sure to do a full complete save with a unique name at least once per gaming session. Think of it as creating your own restore points as you go; that way, you'll never lose more than one day's worth of progress.

Best of luck to you.
Personally I never use quicksave and quickload. The chance of getting a corrupted save is slim, but it happens often enough that I find it not worth the risk.
Forgot to mention: DO NOT STEP ONTO THE ALTAR.

It is the final part of a complex and mostly hidden side quest that, if followed to the letter, grants you a number of bonuses amounting to near-godhood for the rest of the game. If not followed to the letter, your PC dies. (There's also a rumored glitch that if your TA is too high, it won't work even if you've followed all the steps correctly. I can't confirm this, as I've never played a high-TA character all the way through to that point.)

That was indeed the problem. Not one but 2 or 3 previous save files were corrupted. I had to go through all the levels in Vendigroth underground city. Then everything worked out.

I have downloaded the patch but I didn't know you could autosave :D All my save files were manually saved. Maybe I've accidentally pressed f7.
