So the first thing I tried was installing the vanilla GOG version, but it was crashing to desktop after a second. I've also tried to mess with compatibility modes, running as admin and .ini settings, also with no luck :(

Any suggestions what should I try next? I'm running Surface Pro 7 with Windows 10 Home (latest drivers, updates etc.)

I cant offer too much help with this myself, but I can at least confirm that it dose indeed run on windows 10. I installed it from galexy and it worked straight away for me. (i didnt mod anything either) so its likely a conflict with hardware or drivers.
(if it helps i have: i-9700k and a (asus) rtx2080s).

I tried, it's included in the essentials pack - doesn't run with or without it :<
Where did you install Arcanum? If you installed it into the program files you will run into all kinds of issues. If you did, uninstall the game and make a games folder on your drive of choice outside of program files and try installing it there.

I seem to be having similar problems to other folks but despite trying the UAP 2.0.2 patch and adjusting the resolution many times, installing to a folder that is not within the program folder, running in windowed mode, running in 'safe mode', etc, I am still not able to get beyond less than a second into the game. I'm running Intel iris xe graphics with the most recent drivers (30.0.100.9684).
