I just fired it up as a test. No problems.
Win 7 64 bit.
If you run it 4:3 on a 16:10 monitor it has "black" (actually, a wallpaper) borders. It does support 1920x1600 out of the "box".

Text IS a bit blurry, it's not optimised for LCD screens it was designed for CRTs and looks far better that way.
The text is readable, just about, though. I'm afraid you'll just have to live with it.
As for scaling issues, I haven't noticed any... and certainly nothing that impacts my gameplay.

There are scaling problems. And you can obviously note them but you don't want to. And I don't know why.
Example: I've set my resolution on my CRT to 1024x768 and the text is totally illegible.
Because the text font is a cursive one it's hard to read. Even at 640x480.
And certainly impacts my gameplay not being able to play with ease.
See the image I'm attaching.
In the menu it's readable but in the game were the text go away with the voice over it's difficult to read. Even more because I'm not a native speaker of the english language.

Have you tried 800x600? It could be that it looked weird at 640x480 because of bad downscaling.

Creating the file "Config.ini" and writing in it [video] and then ScaleMode=2 doesn't do very much apart from not stretching to full screen but will create nevertheless the same disturbing graphic artifacts.
