Umh....click Install now again?

Before doing this, of course, log out from GOG, clean your cache and try again.

If you did download it, find out where you downloaded it to, and run the installer from there.
If you used the downloader app, view options to see the download directory.
Or if you downloaded it via browser, see your browsers download directory.
i think i hit cancel when it says do nothing it does nothing now it wont download

1) You can always grab the latest version of the GOG Downloader from here
2) You do not need the downloader to grab your games' installers. Switch "Downloader Mode" to OFF, and you will download the games through your browser. Either option should work, but both are known to have occasional hiccups.

I see in your screenshot that you have the GOG Downloader enabled. I guess this means that you used it to download your game, yes?
Do you have the GOG Downloader still running? Does it show the game installer you downloaded (as completed)? If yes, then next to your downloaded game (on the right) you should see a magnifying glass. Clicking on it will open a window to the directory your game installer got downloaded to. Double click on it to install your game.

Keep in mind that you don't need the GOG Downloader to run in order to install and play your games.

Ok, you have the downloader up and running, time to try and add the games to it.
First (obvious) attempt is to click the "Windows Installer, English" button from the first screenshot again. If the downloader picks up the click, all set. If it doesn't, it means your browser isn't passing the click as it should. Telling us your browser should help us help you, though for now you can add Caesar 3 by running the following command (win-r to open run dialog)

"c:\program files(x86)\GOG.com\GOG.com Downloader.exe" gogdownloader://caesar_3/installer_win_en
This tells your computer to run the downloader and pass the caesar 3 english windows installer as a parameter.

edit: proof read the instructions JMich, else how do you expect them to work?

Edit: Saw your first screenshot seems like you're still trying to download the game, please follow JMich's instructions which seem like the most appropriate course of action.

Chrome does seem to have some issues registering the gogdownloader protocol, but support is aware of it and working on it. I am using Iron myself (a googleless chrome) and I don't have trouble, but we did spend a few days trying to fix this very error for another GOGer and failed.

So for now, passing the info through a run prompt seems like your best bet :/
Or you could use Internet Explorer, which should be available on your windows machine, though not sure if you find that a better solution.

Edit: Or alternatively, switch "Downloader Mode" to off and grab the games through your browser. If a game installer has multiple parts though, you need all of them to install it.

No worries.
Look on your keyboard. On the bottom left side, between the Ctrl and Alt keys there should be a key with the windows logo on it. That is usually referred to as Win key. Pressing Win and R together should open up the run prompt (see run.png). There you can add a command you want windows to run. So, the command to add Caesar 3 to the downloader is the one mentioned in the previous post, meaning:

"c:\program files(x86)\GOG.com\GOG.com Downloader.exe" gogdownloader://caesar_3/installer_win_en
This makes 2 assumptions of course, 1) that you didn't change the default installation folder for the downloader and 2) that your Windows is 64-bit and English.
The command has 2 parts, the first one (between "") is the program to run, in this case the GOG downloader, and the second part is what parameter to pass it, in this case the caesar 3 installer. Assuming this does run, we can move on to how to find the correct parameter for other games and/or extras. Nothing hard, though clicking would be easier.
