Audition (1999): Directed by Takashi Miike (Ichii the Killer, mentioned previously, also good movie, not nearly as disturbing, to me, anyway). Its about an older Japanese businessman who agrees to his friend's idea to hold an "audition" for a new wife, pretending it will be for a film role. The person that he picks turns out to be so very disturbed.
This is the only movie that has ever made me uncomfortable. I wouldn't say its scary, per se, but then as an adult I cannot think of any movie that has scared me. Made me jump, sure. Made me nauseous, sure. But scared? As in afraid to watch? No. But this movie makes me very uncomfortable, and it is very well acted, directed and is very engaging. Just see if you can watch the last half hour or so without looking away from the screen or at least wincing.
[Edit] Oh, and The Human Centipede is absolute garbage. The director is a hack and his "films" are the equivalent of eating ramen noodles when you are hungry. Okay, maybe rotten ramen noodles.
