Audition (1999): Directed by Takashi Miike (Ichii the Killer, mentioned previously, also good movie, not nearly as disturbing, to me, anyway). Its about an older Japanese businessman who agrees to his friend's idea to hold an "audition" for a new wife, pretending it will be for a film role. The person that he picks turns out to be so very disturbed.

This is the only movie that has ever made me uncomfortable. I wouldn't say its scary, per se, but then as an adult I cannot think of any movie that has scared me. Made me jump, sure. Made me nauseous, sure. But scared? As in afraid to watch? No. But this movie makes me very uncomfortable, and it is very well acted, directed and is very engaging. Just see if you can watch the last half hour or so without looking away from the screen or at least wincing.

[Edit] Oh, and The Human Centipede is absolute garbage. The director is a hack and his "films" are the equivalent of eating ramen noodles when you are hungry. Okay, maybe rotten ramen noodles.

awalterj: Pan's Labyrinth: Not a horror movie by any means but with some changes this could have easily been turned into a grade A horror movie. The child eater scene would have given me nightmares as a kid.
Seeing this as an adult I'm incredibly amazed by the beautiful art direction but if I had seen this movie as a child, that would have been premium horror material. Seriously, I would not show this to any children who are even remotely imaginative.
I almost mentioned this but like you said, it's not really a horror film. This ranks in my top 5 movies of all time for all the things you mentioned and so much more. It's basically a retelling of "Kiss of the Spider Woman" and I happened to first see it when I was studying that in a 'Magical Realism' class, so the whole thing really hit me like a ton of bricks. It's an absolute must see movie period, regardless of genre.

tinyE: There really is no gore in the Texas Chainsaw. I think the hand slice near the beginning is it. I'd like to pin that on Hooper's artistic integrity but he fully admits one of the reasons there was no gore was that they couldn't afford it. :P
I would like to enjoy TCM. I appreciate its artistry, the lack of on screen gore is a neat trick to make you think you've seen something so much more horrible, but I cannot stand this movie. One reason: the annoying female lead who literally screams her damn head off for the final half of the movie. By the end I was cheering for Leatherface to chop her up. I don't think I was supposed to be cheering for the chainsaw-wielding psychopath. :)

I hear a lot about good Japanese horror movies, but do not speak the language and do not like subtitles :(

+1 to Dagon and Audition; those are great.

I also dislike The Human Centipede; I can handle the wet splatter/gore/scat but it doesn't offer much more than an overused crutch of a thin gimmick.

1) Hellraiser! The entire series, of course. Even though the first was, and still is, the best.

Cult classic.

Hey amok, you seem to know and maybe it's already been mentioned, but I recently saw a Japanese horror film about a guy that reads headlines of tragedies that are about to occur. It didn't rank up there with the best I've seen like the original 'Eye' and several others, but there was this great 'car hitting pedestrian' scene that was so well done in it's surprise and "gory and disgusting without actually being gory and disgusting" way it was filmed. The movie has always stuck with me because of that.

hmm... not sure. He was reading headlines about things to happen, he was not writing them himself? Do you know how old it was (aprox).

edit- it rings a bell, but the one I was thinking about was writing things that comes true (not Deathnote, and I do not remember the name)
