Are they passive? Does it make a difference if I click Use > Pugilism during a fight, for example?

Quite passive. Brawling is your skill with melee and hand to hand both, to some degree, while pugilism impacts your fist damage. As Brawling goes up, you get more attacks in a round. As pugilism goes up, you get more damage from fists.
