If I move mouse in game it feels "laggy". It feel like when your optical mouse something blocking the sensor the movement becomes "jerky" or "laggy".

Is there a fix or is this normal?

For me, I had previously edited dosboxULTIMA1.conf to the following setting in notepad

[SDL]
sensitivity = 200
Interestingly, for a more recent install of Ultima Underworld I had switched to DOSBox Staging and didn't need to do that tweak. Huh.

Anyways, maybe the above fix will solve your issue.
