I had never "died" in this game until now. I was given a special quest by the wizard in town, who game me a map scroll to use in the quest. I used the scroll, went into the dungeon and was promptly squished by some slamming walls (no Speed spell).

I had no idea that the reincarnation in town with no penalty option would remove the scroll from my inventory AND remove the portal to the relevant dungeon, but they are both gone. Is there any way to recreate this, or am I just hooped for the wizard quests for the rest of the game?

Seems like an awfully nasty penalty just for exploring the game options...

Abandon the quest, and pick up a new one when it's available. You shouldn't have that much trouble getting a new one.
You get a few of these per game. They can be very hard and on unique maps you won't see anywhere else. They can be loot heavy, but otherwise there's no loss in failing to complete them.
As the map scroll is a quest reward, it does not affect future quests.

I don't know of a way to reaquire that particular map scroll and complete that map.
I hated the sliding rock walls. Won a few, lost a few. Just have to do another quest for the wizard.
