As the Title says Witcher 3 crashes mostly after some fast travels in-game - i already made this check up with gog galaxy client & co.

Try standard steps:
Reinstall Witcher 3 (delete config files as well, they ar elocated here: C:\Users\<username>\Documents\The Witcher 3)
Reinstall GFX drivers
Check your drive for errors.
