I use Galaxy, but always manually install the game. I previously installed manually (main 1.10 installed first, then patched to 1.21, then installed all the DLCs including Hearts of Stone and Blood and Wine) and had Galaxy verified the game.

When Galaxy prompted me to update the game after 1.22 came out, it said not enough disk space (I had 13GB free space in my OS drive). So I decided to manually install the update. I did see that there is the new 1.10 to 1.22 patch for the main game, and all DLCs are version 1.22. After only downloading the 1.22 patch (2 files, 2.1GB in total), I did the update, and the patch program said it would patch the main game as well as all the DLCs. Without installing the new 1.22 DLCs, I let Galaxy verified the game, and it didn't need to download any additional update and recognized the game as 1.22. I went in the game and the version says 1.22 and all seem fine.

My question is: is the 2.1GB 1.22 patch the only thing we need if we had the the main game and all DLCs installed? Is it true that you don't need to download the full Hearts of Stone and Blood and Wine 1.22 installers? (It appears to be the case for me anyway, and if that's the case, people really just need to manually download the 1.22 patch without downloading 11.2GB DLC installers)
Works for me after only applying the patch over 1.21 game and DLCs.

The 1.22 DLCs are changed however, the install files have a slightly different size. But it appears the patch updates 1.21 DLCs just fine, game runs normally.
Right. It will state when installing the 1.22 patch that it will also patch version 1.21 DLC's and Expansions. You only need to update everything if you hadn't done the update to 1.21 for DLC's, HoS and B&W
It did not work for me. Applying 1.21 to 1.22 patch did not change game version. It still says it is 1.21. I cannot run patch again as it in turn says that it is already downloaded.

When trying to verify in galaxy it still wants to download whole 15GB of data. GOG Galaxy is just bugged it appears. I think i'll stop using it.
I would say that is a good idea. KISS principle.

Same Problem here Bro. And I can't get the update from GOG Galaxy, since its down. :(

ps. There is no patch from 1.21 to 1.22 on the download page for me, it's name is 1.10 to 1.22 (2.8GB) and I have the problem now you stated here.

I'm having the same issue. Any progress?

Apparently instead of updating the DLC to version 1.22, GOG just renamed the 1.21 DLC files to 1.22, causing various bugs in the game to occur. I've notified them via PM... so now we wait.
It was just a tag file that did not get updated the files were fine supposedly.

I used the 1.10+ to 1.22 patch on my Game and DLC with HOS and BAW all at 1.21

Patch took a few mins and stated it was updating Game, DLC HOS and BAW.

Worked fine.
I've been trying to figure this out too. I don't use Galaxy.
I poked around GOogle and the CDPR forums a bit and didn't find any solutions or explanations, but we're not the only ones with graphics problems that are identical. It's widespread. Hopefully CDPR fixes it in 1.23 or 1.3 or whatever is next.
My Witcher 3 graphics problems appear to be gone now also after doing the following steps I outlined in a post in this other thread:
