I loaded the 1.22 patch, and the 2 DLC packages to my PC this week. I see a number of undiscovered locations around Velen and more secondary quests. However most of them are useless. I have gone to a number of bandit camps and since there are no bandits there, ever, I just pick up the stuff. Abandoned sites though aren't clearable though because of this. There are no monsters to clear so it can't get taken care of. Can't complete Surprise inheritance because there is no monster and cannot interact with cellar door even though I have the key. Tried to do the undiscovered laction east of crane cape but can't interact with the ladder to get out of the cellar once I go down to progress the quest. This is so buggy it is almost unplayable at this point.

It's not buggy, it's your installation: it's corrupt. Verify/repair or uninstall/re-install.

Hickory: It's not buggy, it's your installation: it's corrupt. Verify/repair or uninstall/re-install.
You were absolutely correct. Didn't think I would need to do a verify right after doing the install, but this fixed the issue. Thanks!
