It seems I have painted myself into a corner here.

I couldn't find the sewer entrance to save Phillipa and so I chose to get arrested.
I see what is done to her, have a talk with the King and then am killed by that Nilgardian horses rear.

Being killed at this point doesn't make sense to me.
Am I choosing the wrong dialog options, did I pick a fight with the wrong people( insulted the four dice players) or am I supposed to die?

I gave up that and found the sewer entrance and was able to progress as normal.

That's weird, the quest specifically stated to either get arrested or find the sewer entrance. I went the sewer route myself.

Maybe you failed a persuasion/hex/intimidate or something?

I think you mean the part where you got those bindings around your hands. You need to use the igni sign (button bash left mouse) so they melt and you can grab the sword from one of those Nilfgardians.

yeah, there is a QTE here, it's not hard to miss, unfortunately. But the result after you do get it is quite satisfying. I think it was just a left click repeatedly till the bar fills kinda deal.

Yes that the place and I totally missed the QTE, I guess I was busy looking at something shiny instead.

Just another stupid QTE that probably shouldn't have been in the game.

I think the sewer route is more interesting, anyway - and you get to keep your equipment!
