This looks excellent!

I would love to pick this up and put on my 4 player arcade machine -
But it uses an iPac 4 usb interface by Ultimarc -

Does Terror of Hemasaurus include mappable keyboard support for local play?

Or just controllers?

Yes it does have keyboard support, though I personally use a controller so I cannot vouch for how good keyboard controls are for this game.
