I have returned to Lenele. Everyone is dead and the undead populate the city. Found the entrance to the sewers and proceeded to the exit point (yellow line on ground). When I cross the line nothing happens.

How do I exit the sewers?

That's the wrong exit. You've found the exit to the Old City section, this is cut off. Instead you have to head through and take the exit to the Marketplace, on the other side, where you came in first time you entered the sewers..
