Do the occasional smoke puffs coming from other flight members' planes mean anything? As far as I can tell, since the status after flight is usually OK, it doesn't but, perhaps it is random engine malfunction?
The random smoke from just flying is your engine burning oil that gets mixed in with the petrol. An aesthetic thing for atmosphere in the game. However, if the plane has taken some hits and starts coughing smoke on a more regular basis, then worry about an engine fire.

There is a mod so to speak called reload for the SP side of RB3D. It lets you set up various variables and effects. One of the thing is random engine malfunction. If you don't watch out, the engine dies or catches fire while flying.

Some of what it does is:
Set a general, constant wind direction and speed so you can be blown across the map as you dogfight..
Give engine management, including no throttle control for rotaries if you wish.
Make it hard to reload/unjam (must fly straight and level and hit the U key numerous times)
And a bullet counter so you know how many rounds you have remaining before you dive on that red Fokker Triplane.

I think I'll wait a while before trying that one - I'm still a novice at best in controlling my plane and use "Pause" far too often. And don't even mention formation flying ---- :)
That last one, the bullet counter would sure be handy, though. Seems the pilot would have some indication by site as o how many bullets are left in the drum.
For formation flying, you can always use ctrl+a, of course the game will then also fight for you too. alt+a and you will fly from way point to way point. shift+a and you fly in a straight line. to get out just press a.
Use "m" to see your squadmates instead of just friendlies or enemies.
If you ever lead a flight, ctrl+h will tell your flight to go home.
As to formation flying, I can kind of do it in IL-2 flying the big bombers, in RB3D it impossible unless flying in a straight line.
