Key bindings, difficulty etc.. get reset on exit.

Are you using Windows 7? W7 sets things like config files to read only so any settings are temporary. You'll need to right click the config file, in this case I believe it's Postal Plus.ini and right click it and select properties. At the bottom uncheck read only.
