Do I need to install the graphical ddfix even with the most recent installer? I tried running the game by itself and, while I couldn't test any spell, everything appeared to be smooth.
once I install the game from the installer, do I need to run something else? mods and UI alterations don't count.

No. ddrawfix is now installed by default on the GOG version.
