So I'm pretty sick by low quality weapons I had in early game (I'm currently roaming Sigil after escaping Mortuary), that I use TormentHack 1.8.1 to add Battle Axe of Quality into my inventory. I set Item Amount to 1, and Item Count to 0. I got "Item hack successfully applied" message, but when I run the game, I just couldn't find the item in my inventory.

(1) why did it fail?

(2) will it ruin my savegame? I mean, will it cause some serious bugs in the future? For example., bug that won't allow me to finish the game.

I don't know enough about TormentHack, but I do have two questions for you. What OS are you using? Where have you installed your game? If you're using any Windows newer than XP (and to a certain extent XP) you should *not* install to any directory named Programs or Program Files. The OS protects files in these directories to a ludicrous extent.
You neglect to mention which mods you're using as well. If you could also provide a link to this tool, that would be helpful.
Personally I haven't ever been able to get TormentHack to work AT ALL with PST. It's possible that it's due to some strangeness with the GOG version of PST, because Near Infinity can't read GOG's Torment.gam save files, and Near Infinity can read everything.

The way I ended up adding items into my savegame was through editing floor pile container info in the .are files in Torment.sav using NI, but that's a very roundabout and inelegant solution.

So, I grabbed NI, and tried doing the loot pile editing, but I always get an error message saying "Error reading dialog.tlk".

How do I get around that?

I have the big list of mods installed that were recommended on the GoG page.
I managed to find this post looking for help, but eventually figured it out myself.

So, I used the command line version because the GUI one kept on giving errors and then crashing, but regardless, you can't just add an item to a characters inventory.

You can however change one item into another.

In the command line I was able to put:

C:\Folder\TormentHack C:\Torment\SaveFolder\TORMENT.GAM ITEMS PCNORDOM HEARCHRM 1 1 AEGIS

To replace a Heart Charm with an Aegis of Torment instead of trying to figure out where I resurrected Nordom and forgot to loot his corpse :P

Hope this helps anyone trying to find this.
