This same combination of outfit, outfit color, hair, and glasses is generated way too often for female operatives. All but 3 of my female operatives look like this, and just about every time I see a female enemy operative, it's the same clone. There were three of them in one mission tonight. There are a lot more options available in the character creator for it to be generating this same look 95% of the time.

Actually, I've had that very same operative duplicate herself for me as well! I think my game still has three of them, and one of the enemy agents (who we sent back awaiting instructions) also looks exactly like that - makes me inclined to avoid activating her (or if I do, not care too much about her welfare).

It doesn't seem to happen with any of the other female ones, though I did end up with a pair of duplicate guys at one stage - but that was right near the end of the game as I was capturing/brainwashing every enemy on the map just for laughs.
