Xpadder MUST be started AFTER the game is already loaded up (alt-tab up and then start it). If I start Xpadder first then I will get a bug where UP doesn't work or it will spam it and menus will be impossible to navigate. Loading Xpadder 2nd works perfectly.

Please Lord Gog, fix controller support for this game. I don't want to screw around in Xpadder, and feeding in the joystick x/y for the arrow keys doesn't give a good play experience: it sucks, it's not fluid. I want them to fix it!
I might just have to sadly give up on this one. :(

The other option is to download a Dreamcast emulator and try to play it through that; you'd definitely have perfectly-functional controller support with NullDC or Demul, using an Omikron ROM. It's a really disappointing port GOG, you guys screwed this one up. It's sad really because this is a fantastic hidden gem of a game. Sure it has some flaws, but if you can see past the couple wrinkles it's an absolute masterpiece.

GOG! Please fix the controller support for this game, I don't want to be downloading Dreamcast emulators and pirated Dreamcast ROMs but it looks like I have to in this situation due to this messed up port. :(

Did you have tryed the switch on the back of the controller?

I'm playing it on steam but even whit direct input controller I can't play :(

Someone wrote a DirectInput wrapper mod for Omikron - namely for X360 controller support.
