Not sure if my memory from 20 years ago is failing me, but I was certain you could do no kills with the usage of stun grenades and CQC in forced encounters.
I'm in the first scripted fight where the guards burst into the room with you and Meryl and they just will not get KO'd from anything after literally dozens of strikes.

Am I just misremembering here and confusing all the later games' non lethal runs?

Impossible in MGS1.
You still have to kill few in order to progress story - 3 in cell, 4 in elevator (comm B), 3 in cargo elevator, 2 + 2 + 3 in escape route, and few extra depends on how game play goes. So likely you need to kill minimum 19 or 20 no matter how you play.
You can finish game without saving once.

Yeah I thought some of the Bandana/Stealth unlocks were tied to no kill ranks; but I guess that all started with MGS2 onward. Been way too long.
If I recall it right, getting the Bandana or the Stealth Camouflage depends on wheter you resist torture or not. If you resist torture, you'll get the Bandana and one of the two different endings of the game. If not, you'll get the Stealth Camouflage and the other ending. That's all I will say about those endings. I'm trying hard to avoid any spoiler ;-)
