The game is very likely not starting because you don't have a CD-ROM drive.
(yes, you need a CD-ROM drive - and it doesn't matter if a CD is inserted)
Quick solution: Get and enable kqmoefix: http://kqmoe.nicode.net/
(the CD-ROM emulation is enabled by default - and it doesn't matter if the directory exists)
