Either something was changed in the latest patch or there is a bug, because Minfred no longer drops Decapitator. Whether I have the cursed necklace quest and he kills himself, or if I only have the Decapitator quest and I kill him, he only drops a random sword. What's up?

I don't know what's going wrong, but if you aren't able to fix it game wise, as a last resort you can use the console to spawn the item:

Press ~ to open the console.
A prompt will appear near the top left of the screen and you can type in the following:
\givenewitem relikvie_Smrtihlav

Thanks! I did however find the sword, it was in the hands of Wulfhyrr in the wolf den, there is even a dialog choice for it.
