I have never found a site that doesn't offer the same old tired, recycled (and sometimes blatantly ripped off) portraits. I make my own.

Exactly, most of the time I find everything I need on www.deviantart.com. From the top of my head, here's a nice pic I recently used for IWD:

Also, you can "icewindalize" any portrait using photoshop and this handy tutorial:

Though IWD has really good default portraits (especially compared to ridiculous BG ones).
