is all the dlc and addon included in the patches. or do I need to install separately?

The Addon is not included now. You have to buy and download it on release. For the DLCs it is the same. Buy the free DLC pack in store and download them if you want them.

no I mean all the free 16 addon dlc. the new quests like fools gold. not the paid dlc that's coming

O.K. See my first link. There are all information you need. They are not included in the patches you have to download them one after one.

If you use Galaxy you can download the DLC by clicking More -> Configure.

Note that the extra content (quests, sets) will be active in-game automatically without any further options. The new outfits and Gwent card designs can be turned off and on in the main menu of the game before loading a save game or starting a new game.
