i7-9700 3ghz
Gtx 1660Ti
16GB Ram

I have deleted the .ini files.

I have changed compatibility to Win XP Svcpk 3 and run as an administrator.

I have the resolution set to 1920X1080.

The game won't run unless I use FOSE and NVAC mods.

I just get a black screen with the starting music but the mouse and keyboard do nothing and I can't alt-tab out.

Any ideas or do I need to refund to GoG?

Have you limit your CPU-Cores for F3?
Fallout 3 can not handle more than 2 Cores.

Open the 'FALLOUT.ini' and search for the entry: General.

Location of the .ini:
USERNAME\Documents\My Games\Fallout3

Add these two lines directly below section [General]
It should look like this:

[General]
bUseThreadedAI=1
iNumHWThreads=2

(if "bUseThreadedAI=0" already exists, change the 0 to 1.)

Close and save the file.
