All the keyboard controls do not work. The "+" key does not increase thrust. The page up and page down keys turn the aircraft, not change or alter waypoints. There are additional keys that do not work as the key card states. Has anyone else encountered this problem?

try using the the plus key next to the backspace key, and as for the PgUp and PgDn keys - if you're using those on the numpad, try pressing NumLk first.

Thanks for the response. If I hold down the num lock and press the pg up and pg down it works. However, the plus key still does not work. I have pressed every key and none control the thrust. Stuck on the runway and cant take off.

That is really strange - perhaps you should check in notepad if the button works? Its the button with the '=' and '+' on it. If you press it once it increases the thrust by 10%, and if you hold SHIFT and press it then it increases the thrust to 100%.
Also, I don't think you have to hold numlock down - if PgUp and PgDn do not already work then it should be enough to simply press NumLk once.

Laptops often have incompatible keyboards for games especially flightsimulators that use a lot of keys.
Do you have a spare keyboard laying around that you can hook up?
If not then you're probably have to rebind the keys with Dosbox keymapper.

Thanks. The key mapper did the trick. Remapped some of the key and now it works.
I am having the same issue (see other thread)...but I cannot resolve it using the above suggestion. Everything besides thrust works just fine, which is really odd. I'm using a normal keyboard on a desktop computer, not a laptop. Has anyone any other suggestions by chance?
UPDATE: Found a fix that worked for me.

I can't post the specific link for some reason - guess the forum prevents that. In any case, the site had a long list of information, but the specific bit there got everything to work for me:

Actually, rather than disabling the device, there is another workaround if you want to keep using it.
In Device Manager under Keyboards, update the "Microsoft eHome MCIR 109 Keyboard" to a "HID-compliant device"

Restart the game if you had it open, and presto, now you can type tilde's and still use your remote.

This is a very old and known problem that existed already in the DOS days.

You have to change inside DOSBOX the keyboard language layout to US layout, then the key for thrust will work.

Just start DOSBox and then in command line enter:
keyb us

You can also define a US keyboard layout in the DOSBox configuration file.
