Hit a bug I haven't seen before. Went in an ran Dagolar as normal after the Sewers. Got through the Door of Eyes per the usual way. Brought a friendly Zombie with me to face Dag, and fought him and his happy slimes in a hit and run battle around his bedroom. After I finally dropped him, as I'm walking back to the exit - the Door of Eyes closes on me when I get close. Spent the last couple of hours trying to figure the scripting. There's a number of paths to get back to the statue room. When my lead character get's just south of Zombie room (south) or, just north of the torture chamber north of the zombie room (past the doorway to the room with a chessboard) (middle), or as they walk past pool of water north of the statue room (north) - the Door of Eyes closes on me. It's only triggered by my lead character - if I separate the party and one of the group decides to take a weird pathing to get to where it wants to be - it doesn't trigger the Door of Eyes.

Once closed - the Door of Eyes states it cannot be opened on use. Attacking doesn't do anything.

Tried reloading a number of time, including one where I refought Dag. Doesn't fix it. Tried a reinstall, no dice.

Any thoughts on how to get past this? I don't remember having problems here in the past.

Hmm, I can't say that this bug has ever happened to me. How exactly did you beat Dagolar? Did you actually kill him in combat or did you beat him by playing the organ? To maximize my XP, I usually kill all of Dagolar's summoned slimes, and then play the organ. (You'll get Dagolar's loot normally via this method, so no need to worry about that.) I've never had an issue with getting out of the dungeon via that method.
