I'm up to the quest where you have to convince Bronthion to go to the council of seven, and I need to get the three elven artifacts. I've gone through to the burial grounds, into the cave and I've defeated Josephine and the dwarves. I grabbed the two artifacts and left through the magic door, not realising that the third item was a staff dropped by Josephine herself. it wasn't until I got back to the elven village (a few quicksaves later) that I realised so I returned to the room where the artifacts were. There is no staff there at all. I've scoured the room, the adjoining cave and even the camp outside.
Is it possible for an item to fail to appear? Is there any way of hacking the item back into the game? I have a bad habit of only keeping the one or two quicksaves, so I have no save before the Josephine fight.

Have any unidentified staffs in your inventory identified. Two items are identified but the staff is not. You may have it but cannot complete the quest until it is identified. The staff looks plain with nothing to distinguish it from a regular staff. If you sold it without identifying it, you can buy it back from the merchant if they only have the one staff or you remember what it looks like.
The staff has also be known to fall in a wall where it can be seen but not picked up. If you can see it try telekinesis as a way to pick it up. Press the ALT key to make all item labels in the room appear. Try clicking on the item label, it may allow you to pick it up. If not try using telekinesis to move it out of the wall. Once on the floor it can be picked up as usual.
The last solution is to go back to the save you made prior to the battle with Josephina and do the battle over. The staff should fall normally as the wall thing is rare. . . =)

I have no staffs (Staves?), let alone unidentified ones in my inventory. There aren't any in the surrounding cave/camp that I may have dropped or thrown away.
Yeah, I've heard of the wall glitch. I've pressed ALT, the only item labels that show up are a helm and axe dropped by the dwarves, and the cheese and ale on the table in the back of the room. Definitely none stuck in the walls.
And sadly, a really poor gaming habit of mine is to only save on the one save slot. I didn't realise I hadn't gotten a staff until a bit later, so I have no save prior to the Josephina battle.
I've heard there are item editors for the game, could something like that be used to manually put the quest staff back in the game? I've otherwise been really enjoying the game, and it would be a shame to have the game broken.

I did the same, concerning saves, on my first play through. I haven't tried any trainers or editor but they are available. Using Google use [ "divine divinity" trainer OR editor ] as the search criteria and it brings up several. I can't recommend any and would also say use them at your own risk for obvious reasons . . . =)
Edit: This is the editor I would probably try first as Gamebanshee is fairly trustworthy. I have not used it, don't know if the download link is good, etc etc. . . but I would start here.

Okay....consider this question solved. The Elven Ceremonial Item was sitting in Josephina's room all along. However, It was sitting behind a wall (So the item was completely obscured) and (This is the really weird part) Its name didn't show up when ALT was pressed. I'm not joking, every other item's label appeared except for the staff. I was only able to find it by scrolling my mouse over it.
So I'm able to finish the game now, but I wonder if this has happened to anyone else?

Glad you were able to find it, if not found it was a game breaker. It has happened to others, hence my answer of possible fixes, strange the ALT key didn't show it since it usually does.
