I want to play this like I did back in the 90s with the cursor keys and no mouse aiming but there is no way to bind a strafe toggle button (ALT in the original Blood). Any ideas?

At a hunch I tried editing the %AppData%\Nightdive Studios\Blood EX\kexengine.cfg file to include the key bindings:
bind lalt "strafe"
bind lalt "+strafe"

Unfortunately this didn't work. ;-(

Sadly it seems not to have a mapping to assign a button to it.
